Changing your contact details

To update details like your name, address, phone number or email address:

• fill out a change of contact details form
• phone or visit the surgery from 9am to 5pm

Out of Area?

BEFORE completing a "Change of contact details form", please check whether your new address is covered by our practice boundary  

If the search reveals that you are outside our boundary, visit Find a GP  to locate a surgery that serves your area.

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

• Check you are in our catchment area

You will need to show us proof that you have changed your name or date of birth. This could be a Marriage or Civil Partnership Certificate, Deed Poll or Birth Certificate.
